If you are unable to download the NHS app, please phone 119 for a paper copy of your vaccinations. Please DO NOT phone the surgery as we’re unable to help.
If after downloading the app and accessing your ‘COVID-19 VACCINE RECORD’ you find that some vaccine data is missing or incorrect please email [email protected] giving your full name, address, and date of birth followed by full details of what is missing/incorrect and we will investigate this for you. If you are also able to attach a photo of the vaccination card which you were given at the time of your vaccine that would be most helpful. We aim to respond within 10 working days although this may be longer depending on demand. Please be patient and bare with us throughout this extremely busy time.
We are urging patients NOT to tie up our phone lines by phoning the surgery for vaccine queries. Our staff are not able to help with any element of this over the phone. Thank you for your cooperation.
